Job Summary
Diagnose and manage acute and chronic conditions while conducting annual wellness visits, routine check-ups, and home visits as needed. Build and maintain long-term patient relationships through personalized care plans and health reports, providing after-hours support via phone for patient care. Monitor physical and mental health development and lead a dedicated care team comprising a Registered Nurse and Patient Care Administrator. Collaborate with in-house and external specialists to proactively manage patient health within a limited panel of approximately 100 patients per physician.
Required Qualifications
- MD or DO from an accredited medical school
- Residency completed in Internal Medicine, Family Medicine, or Medicine-Pediatrics
- Minimum 5 years of clinical practice experience
