Job Summary
Conduct annual wellness visits, routine check-ups, and home visits to diagnose and manage acute and chronic conditions while building long-term patient relationships. Monitor physical and mental health development, develop personalized care plans, and provide after-hours support via phone. Lead and collaborate with a dedicated care team including a Registered Nurse and Patient Care Administrator to deliver preventive, longitudinal, and whole-person care. Maintain high clinical autonomy within a collaborative environment featuring a limited patient panel of approximately 100 individuals.
Required Qualifications
- MD or DO from an accredited medical school
- Residency completed in Internal Medicine, Family Medicine, or Medicine-Pediatrics
- Minimum 5 years of clinical practice experience
