Job Summary
Conduct intake assessments and collaborate with members to develop individualized care plans. Coordinate referrals, follow-up appointments, and needed services while acting as the primary point of contact for members navigating their care. Monitor progress and update care plans as needed, maintaining clear, timely documentation. Work closely with the clinical team to ensure whole-person support and share insights to strengthen the overall member experience. This per diem role requires 6+ hours weekly in the DMV area and may grow to full-time as the practice expands. Join a new preventive-first medical practice launching this fall to help shape a modern, prevention-focused approach to health and wellness.
Required Qualifications
- Active license in D.C., Maryland, or Virginia as either: Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN)
- Experience in care coordination, case management, or concierge-style care
- Credentialed with major insurance providers
- Strong commitment to preventive care and personalized member support
- Excellent communication and organizational skills
- Ability to work independently and collaboratively with a small, mission-driven team
- Live In: DMV Area (D.C., Maryland, Virginia)
Desired Qualifications

- Credentialed with major insurance providers strongly preferred
