Job Summary

Conduct comprehensive patient visits in clinic or via telehealth to identify and close care gaps for senior members while managing chronic conditions. Review patient charts pre-visit to develop plans addressing outstanding healthcare needs, interpret diagnostic results, and maintain accurate medical condition lists. Collaborate with physicians and care teams to prioritize scheduling, coordinate follow-up care, and manage routine medication refills per clinical guidelines. Recognize urgent medical situations, educate patients on disease management and preventative screenings, and complete documentation in the EMR per organizational standards.

Required Qualifications

  • Active Nurse Practitioner (NP) or Physician Assistant (PA) license, or ability to obtain prior to start date
  • Current national certification through ANCC, AANP, or NCCPA in one of the following areas: Family Practice, Geriatrics, Adult Health, Adult-Gerontology Primary Care, Adult-Gerontology Acute Care
  • Minimum of 1 year of clinical experience at the highest level of education completed
  • Ability to meet physical requirements of the role, including lifting up to 30 pounds, navigating various environments, and performing physical assessments
  • Willingness to travel within the licensed state when needed
  • Experience working with electronic medical records (EMR)
  • Strong understanding of ICD-10 coding and clinical documentation
  • Knowledge of diagnostic criteria and evidence-based medical practices
  • Ability to develop, implement, and evaluate individualized patient care plans
  • Strong clinical judgment and ability to work independently
  • Excellent communication skills with patients, families, and healthcare teams
  • Ability to work effectively with diverse patient populations and healthcare professionals

Desired Qualifications

  • Experience working with senior populations preferred
  • Background in gerontology, cardiology, internal medicine, endocrinology, or primary care preferred
  • Familiarity with Medicare Advantage, HCC coding models, and quality measures (STARs) preferred
