Job Summary
Complex Transitional Care Nurse needed to provide comprehensive care coordination for individuals with chronic or complex physical/behavioral health and social determinant needs. Responsibilities include coordinating care across multidisciplinary teams, monitoring care plans, conducting discharge/transition planning and clinical interviews, reviewing records, assessing barriers to treatment, and referring to available services. Required Qualifications
