Job Summary
Coordinate wraparound support plans for a caseload of 7-10 highly complex individuals experiencing homelessness, unstable housing, and multiple connected challenges. Provide ongoing case management, relationship building, and triage to help service users engage with services, stabilize, and settle into accommodation. Liaise with Belfast Complex Lives Multi-Disciplinary Team partners and convene multi-agency meetings to agree actions and ownership. Organize key worker sessions focusing on responsibility-taking, reflection, and action planning to overcome crisis and trigger points. Manage progress against outcomes, update support plans, and escalate issues to the MDT as necessary. Ensure Health and Safety standards are met, complete relevant checks, and maintain accurate client records and financial transactions. Adapt to new ideas, share workloads with colleagues, and support volunteers while maintaining a high quality of work aligned with Depaul values.
Required Qualifications
- A formal Level 3 qualification in a social care field, e.g. diploma in health and social care or equivalent
- At least 2 years paid experience working with vulnerable groups
- A full driving licence
- Full access to a car insured for business purposes
- Willingness to undertake regular travel in the local area
- Access NI
- Valid NISCC registration
