Job Summary
Conduct intake and psycho-social assessments for individuals facing substance use disorder, mental health challenges, and homelessness. Serve as the primary point of contact for LEAD referrals and respond to law enforcement for warm handoffs. Develop tailored case management plans, document activities in the case management database, and maintain collaborative relationships with housing providers, service agencies, and law enforcement officers. Provide street outreach to engage participants and deliver training to law enforcement and community partners. Shift work, nights, weekends, and holidays may be required, with on-call status possible.
Required Qualifications
- Must have a minimum of a high school diploma/GED
- Must be able to earn the required certification within 18 months of hire
- Level 2 Must have a minimum of 3 years of Complex Case Management experience
- Must possess a valid driver's license and be insurable
- Must be able to stand, walk, and sit (including driving) for extended periods of time
- Must be able to respectfully engage with people experiencing homelessness, substance use disorder, and mental health disorders
- Willingness to follow harm reduction principles
- Experience in data collection, case management, and clients struggling with SUD
- Experience developing safety plans
- Ability to work with diverse populations in various states of anxiety, sobriety, and aggression
- Ability to verbally de-escalate individuals
- Ability to visually assess for mental or physical health crisis
- Ability to visually assess for intoxication
- Ability to quickly problem-solve and make decisions
