Job Summary
COMPASS Nursing Case Manager – Per Diem (Part-Time) in Exton, PA. Provide in-home nursing and case management for geriatric patients, 1-2 days per week (8am-4pm), on-site with remote collaboration. Responsibilities include coordinating care, documenting and assisting with orders for testing, medications, referrals, health information in the chart, vital signs, hands-on patient care during visits, medication reconciliation, coordinating referrals and telemedicine appointments, HIPAA compliance, and effective communication with the care team and PCP. Requires LPN or RN license, 4+ years of similar experience with geriatrics, proficiency in EHR, and telemedicine capabilities. Location: Exton, PA, US.
