Job Summary
Care Navigator (Compact Licensed Physical Therapy Assistant) working fully remotely to support patients in musculoskeletal rehabilitation through remote monitoring and a home-exercise program. Responsibilities include remote monitoring to guide patients through therapeutic exercise, neuromuscular re-education, and functional training via the company platform; provide ongoing feedback to evaluating clinicians on patient progress; maintain records; help improve platform processes; organize and manage a caseload; and demonstrate empathy, strong communication, and data-driven decision making. Requires graduation from an accredited physical therapy program and an active PTA license in a compact state; 2+ years of musculoskeletal health experience is a plus; must be tech-savvy with proficiency across EHR, messaging, and scheduling systems; ability to work independently in a fast-paced startup environment.
