Care Navigator role supporting patients remotely through home-exercise therapy, including remote monitoring, therapeutic exercise guidance, neuromuscular re-education, and functional training via our technology platform; responsibilities include maintaining records, providing feedback to clinicians, improving platform processes, and managing a caseload; candidate should have 2+ years in musculoskeletal health, PT license in a compact state, tech-savvy with multiple platforms (EHR, scheduling, messaging), strong communication and patient-focused skills, independent work style, and ability to work in a fast-paced startup environment.
