Job Summary
Care Navigator role for a licensed Physical Therapist delivering remote monitoring and guidance for patients on a home-exercise program. Responsibilities include directing therapeutic exercise, neuromuscular re-education, and functional training via our technology platform; providing ongoing feedback to clinicians on patient progress; maintaining records; and improving platform processes. Requires empathetic, patient-focused engagement, strong communication and data-driven decision making, and ability to manage a caseload remotely within a fast-paced startup in musculoskeletal health.
Required Qualifications
- Graduated from an accredited physical therapy program
- Active PT license in a compact state
- Experience with musculoskeletal health (2+ years a plus)
- Tech Savvy across EHR/messaging/scheduling
- Excellent communication and data presentation skills
- Independent and proactive work style
- Ability to work in a fast-paced healthcare startup environment
