Job Summary
Care Navigator (Remote) with licensed physical therapy background, responsible for remote monitoring and guiding patients through home-exercise therapy, providing therapeutic exercise, neuromuscular re-education, and functional training via our platform, and delivering ongoing feedback to clinicians while maintaining records. You will organize and manage a caseload, help improve platform functionality, and engage patients to drive musculoskeletal health outcomes. Ideal candidates are self-starters who are empathetic, process-focused, tech-savvy, capable of building trusting relationships with patients, and able to work independently within a growing healthcare SaaS environment.
Required Qualifications
- 2+ years of experience in musculoskeletal health
- Graduated from an accredited physical therapy program
- Active PT license in a compact state
- Tech savvy; comfortable switching between EHR, messaging, scheduling systems
- Experience in behavioral modification is a plus
- Excellent communication, interpersonal, and presentation skills
- Strong relationship-building and listening skills
- Ability to multi-task and manage various types of users
- Ability to work independently with limited supervision
- Willingness to work in a fast-paced, cross-functional environment
- Passion for health and wellness
