Care Navigator (Licensed Physical Therapist) working remotely to support patients through remote monitoring of home-exercise therapy, neuromuscular re-education, and functional training via our platform. Responsibilities include guiding patient progress, providing ongoing feedback to clinicians, maintaining records, improving platform processes, and managing a caseload with efficient workflows. Requires a licensed PT in a compact state, strong communication, ability to work independently, and comfort with multi-platform technology. Required Qualifications
- Graduated from an accredited physical therapy program
- Active PT license in a compact state
- Tech Savvy with multi-platform experience
