Job Summary
Care Navigator, a remote, licensed Physical Therapist, will provide remote monitoring and guidance for patients in home-exercise therapy. Responsibilities include directing and assisting users in therapeutic exercise, neuromuscular re-education, and functional training via our platform; providing ongoing feedback to evaluating clinicians on patient progress; maintaining records; improving platform processes; and organizing a caseload. Ideal candidates have a PT degree, active compact-state license, 2+ years in musculoskeletal health, tech-savviness with EHR/messaging/scheduling, strong communication, and a patient-focused approach to support musculoskeletal health in a fast-growing healthcare SaaS environment.
Required Qualifications
- Graduated from an accredited physical therapy program
- Active PT license in a compact state
- 2+ years of experience in musculoskeletal health (preferred)
- Tech Savvy with multi-platform EHR/scheduling tools
- Strong communication and interpersonal skills
- Ability to work independently and manage a caseload
- Ability to provide remote monitoring and guide home-exercise therapy programs
