Job Summary
Care Navigator role providing remote monitoring support for patients in a home-exercise therapy program. Deliver remote therapeutic exercise, neuromuscular re-education, and functional training through the platform; provide ongoing feedback to the evaluating clinician on patient progress; maintain records and notes; assist in improving platform processes; organize and manage a caseload while working independently. Requires PTA license in a compact state, graduated from an accredited physical therapy program, and 2+ years of musculoskeletal health experience. Must be tech-savvy with strong communication, interpersonal, and data presentation skills; ability to thrive in a remote, fast-paced environment and to collaborate across cross-functional teams.
Required Qualifications
- Active PTA license in a compact state
- Graduated from an accredited physical therapy program
- 2+ years of experience in musculoskeletal health is a plus
- Tech Savvy
- Experience in behavioral modification is a plus
- Excellent communication skills and ability to collect/present meaningful data
- Ability to work independently with limited supervision
- Ability to multi-task and manage various types of users
- Ability to organize and manage a caseload while staying efficient
