Job Summary
Care Navigator (Physical Therapy Assistant) providing remote monitoring and patient support through the company’s platform. Responsibilities include guiding patients through home-exercise therapy, leading therapeutic exercise, neuromuscular re-education, and functional training; providing ongoing feedback to the evaluating clinician; maintaining records; improving platform processes; and managing a caseload. Requires PTA license in a compact state, graduation from an accredited PT program, strong communication and data-driven decision skills, comfort with multi-platform tech, and ability to work independently in a remote setting.
