Job Summary
Care Navigator role for a Licensed Physical Therapy Assistant (PTA) enabling remote patient support through a home-exercise therapy program. Responsibilities include remote monitoring of patients, guiding therapeutic exercise, neuromuscular re-education, and functional training via the company platform; providing ongoing feedback to clinicians; maintaining patient records; aiding platform improvements; organizing and managing a caseload; and delivering impactful patient-centered care in a remote, healthcare-software environment.
Required Qualifications
- Active PTA license in a compact state
- Graduated from an accredited physical therapy program
- Tech Savvy (comfortable with EHR, messaging, scheduling)
- Excellent communication/interpersonal skills
- Ability to work independently with limited supervision
- Experience in musculoskeletal health (2+ years is a plus)
- Ability to multi-task and manage a caseload
- Passion for health and wellness
