Community Weaver acts as a welcoming central point of contact for patients and families seeking information or resources at Eskenazi Health. The role connects patients to Eskenazi programs and community resources, navigates between departments, and provides specialized service to address social determinants of health. Responsibilities include acting as a community health concierge, solving immediate social needs via in-house resources or community referrals, bridging social capital to connect patients to services, coordinating with Social Workers and Community Health Workers, engaging volunteers as a volunteer coordinator, and supporting staff leadership in neighborhood activities. The role may involve flexible hours including weeknights and weekends. Preferred qualifications include a Bachelor’s degree or a 2-year certificate and Community Health Worker Certification.
