Job Summary
Community Visiting Nurse (RN) role providing safe, holistic nursing care to patients in home/community settings primarily in Markham/Stouffville. Responsibilities include collaborating with patient/family/c caregiver and health professionals to formulate and implement a care plan based on patient assessment; delivering nursing care per the written plan; evaluating and adjusting treatment plans in consultation with the patient/family/caregiver and other health professionals; acting as a patient advocate; promoting patient safety and comfort in the home; sharing nursing knowledge with other health care providers. Qualifications emphasize RN in good standing with the Ontario College of Nurses, 1 year community experience, strong problem solving, time management, crisis resolution, interpersonal and communication skills, IT experience, CPR/BCLS certificate with ACLS/NRP for specialty nurses, criminal history check, and valid driver’s license with reliable vehicle. Bilingual ability is considered an asset. Required Qualifications
- Registered Nurse in good standing with College of Nurses of Ontario
- One (1) year recent experience in the community
- Excellent problem solving, time management, and crisis resolution skills
- Excellent interpersonal and communication skills, both verbal and written
- Experience with Information Technology
- Current CPR (BCLS) Certificate; ACLS/NRP for Specialty Nurses
- VSS within the past year of hire confirming absence of a criminal history/record
- Valid driver’s license and reliable vehicle is a requirement
- Second language is considered an asset
