Job Summary
Conduct healthcare navigation services for enrolled clients, coordinating medical home placements and first appointments while collaborating with a multi-disciplinary team to address social determinants of health. Arrange supportive services and referrals to community partners to improve health outcomes and prevent avoidable hospital admissions or emergency department visits. Provide limited health coaching, advocacy, and assistance with healthcare payor applications, prescription assistance, and benefit programs. Attend appointments and make home visits as required to advocate for clients and connect them to needed resources. Requires an Associate degree in Human Services, Sociology, or Psychology; Bachelor's degree preferred.

- Associate degree in Psychology
- Associate degree in related fields with 5 years of experience in healthcare
- Associate degree in related fields with 5 years of experience in care coordination
- Associate degree in related fields with 5 years of experience in case management
