Job Summary
Conduct Community Living Supports to increase personal self-sufficiency and facilitate community inclusion for individuals with disabilities and seniors. Implement goals from Person-Centered Plans by providing assistance, reminders, observation, and guidance. Provide personal care, assist with vehicle entry/exit using lifts, and drive participants to and from community activities. Record service logs, complete incident reports, and maintain common living spaces to ensure health and safety. Ensure participants are aware of their rights while attending scheduled staff meetings. Maintain a culture of gentleness, dignity, and respect. Requires a valid Michigan driver's license, ability to work in a vehicle, and part-time hours of 20-29 weeks.
Required Qualifications
- High school diploma or equivalent (GED)
- Ability to read and write English
- Ability to perform basic math
- Ability to comprehend written instructions
- Ability to record information
- Capacity to work in a team with intermittent supervision
- Ability to work respectfully and courteously with a wide variety of individuals, including individuals with disabilities, senior citizens, children, supervisors, and other drivers
- Valid Michigan Driver's License
- Good driving record that meets Pioneer Resources' policy requirements
- Willingness and ability to travel up to 5% for meetings, training, and conferences
- Ability to stand, walk, sit, talk, hear, and use hands and fingers to operate a computer, telephone, keyboard, etc.
- Ability to reach, stoop, or kneel to perform miscellaneous duties
- Close visual acuity to perform activities such as preparing and analyzing data and figures, transcribing, viewing a computer terminal, extensive reading, and various visual inspection requirements
- Ability to move up and down the steps located on the bus multiple times throughout the day
- Ability to push and maneuver occupied wheelchair(s) in and out of buses with the use of electric and manual chair lifts
- Ability to assist and use public transportation alongside a participant
- Ability to provide personal care as needed
- Ability to provide safe transportation from the program site to community activities, among community activities, and from the community activities back to the program site
- Ability to assure all participants are aware of their rights
- Ability to provide a culture of gentleness, dignity, respect, and a safe environment to all participants receiving Community Living Supports
- Ability to ensure that all common living spaces indoors and outdoors are maintained to a standard that ensures the health and safety of the residents
- Ability to attend and participate in the Person-Centered Planning (IPOS) process when invited
- Ability to implement goals identified in the Person-Centered Plans (IPOS) and other plans a participant may have by providing assistance, reminders, observation, guidance and/or training
- Ability to assist participants in and out of vehicles, using the wheelchair lift, if required and securing belts and restraints and driving the vehicle
- Ability to report vehicle maintenance needs to the SIL Supervisor
- Ability to report all maintenance needs to the SIL Homes Supervisor
- Ability to attend all scheduled staff meetings and other meetings/In-services requested by the SIL Homes Supervisor
- Ability to complete all required initial and update training including CPR, first aid training, and any other training required for the job
- Ability to maintain necessary certification, education, and physical condition requirements of the Michigan DOT
- Must be willing and able to travel up to 5% for meetings, training, and conferences
- Must be able to lift up to 50 lbs
- Must be able to lift up to 25 lbs frequently
- Must be able to work in a vehicle with varying temperature conditions
- Must be able to work outdoors in a variety of climatic conditions as needed to load and unload passengers or pre-trip and fuel the vehicle
- Must be able to follow any other instructions and to perform any other duties, as assigned
Desired Qualifications
- Chauffer's license (reimbursed if not possessed)
