Community Supports Specialist

Job Summary

Maintain a caseload of 20–30 consumers by conducting comprehensive assessments and developing person-centered Individual Independent Living Plans addressing housing, healthcare, transportation, employment, and benefits. Assist individuals in accessing affordable housing, vouchers, and rental assistance while providing ongoing follow-up to promote long-term housing stability. Navigate Medicaid, waiver services, and public benefits for consumers with complex needs, offering consultation to Navigators on resource gaps. Build collaborative relationships with housing, healthcare, and government partners across the 13-county service area, representing the organization in community meetings and coalitions. Identify recurring barriers to support systems-level solutions. This role requires a Bachelor's in social work and five years of experience in disability services or case management.

Required Qualifications

  • Bachelor's degree in social work
  • Five years of progressively responsible experience in disability services, housing services, healthcare, behavioral health, community resource coordination, case management, or a related field
  • Ability to assess complex needs and coordinate services across multiple organizations and systems
  • Strong interpersonal, relationship-building, problem-solving, and communication skills with the ability to manage multiple priorities using a person-centered approach

Desired Qualifications

  • Master of Social Work (MSW)
  • Bachelor's degree in a closely related human services field and significant relevant social services experience
  • Experience with Medicaid, waiver services, affordable housing programs, public benefits, or community resource systems
  • Knowledge of housing systems, Social Determinants of Health, Medicaid, waiver services, public benefits, and community resources
  • Knowledge of Independent Living philosophy and disability rights
  • Leadership experience
