Provide personalised support to individuals in community and home settings; assist clients with daily living activities, including personal care and household tasks; encourage and support clients in achieving their personal goals and enhancing their independence; maintain accurate records of client progress and report any changes in their condition; collaborate with other healthcare professionals to ensure comprehensive care; adhere to organisational policies and procedures, ensuring a safety-first approach; respect and uphold professional boundaries, privacy, and confidentiality.
Required Qualifications
- Certificate III in Individual Support
- Certificate IV in Community Services
- Diploma in Community Services
- Experience working independently and as part of a team
- Experience supporting older people and seniors in community and home settings
- Experience working with people living with disability and complex needs
- Competency in using smartphones and mobile devices with internet access
- Commitment to quality care and a safety-first approach
- Understanding of professional boundaries, privacy, and confidentiality
