Job Summary
Provide daily one-to-one support to clients to work toward their Individual Support Plan goals, assist with smoking-related safety and cessation resources, assist in risk assessments and Safe Smoking Agreement adherence, maintain client documentation and engage with the care team to develop action plans and non-smoking group activities, while fostering respect for client rights and promoting health and wellness.
Required Qualifications
- High-school diploma from a recognized institution plus related experience
- Valid First Aid-CPR Level C Certificate
- Valid police information check with vulnerable sector
- Effective and professional communication and people skills
- Proficiency with Outlook, MS Word, Excel, PowerPoint and other common applications
- Ability to make effective decisions in challenging situations
