Community Support Team Professional III

Community Support Team Professional III provides clinically-based supports to individuals with mental health or substance use issues to help them achieve personal goals and return to their natural function in the community. Responsibilities include psychoeducation, assisting with behavioral and substance use disorder treatment interventions, developing relapse prevention and disease management strategies, coordinating assessments, developing and revising the person-centered PCP, liaising with medical and community providers, ensuring linkage to appropriate services, monitoring progress, delivering intensive case management, facilitating crisis response, assisting with housing and financial resources, ensuring compliant documentation, and maintaining collaboration with team members. Frequent travel to community locations and other outreach destinations; driving and providing/arranging transportation as required; participate in on-call, and maintain certification and compliance with agency/state/federal guidelines.
