Job Summary

Community Support Team Lead for a full-time role in North Carolina to provide community-based therapy and case management to adults with serious mental health challenges. Lead and supervise CST services, conduct initial assessments and reassessments, deliver individual/family/group therapy using evidence-based practices, develop person-centered plans, monitor symptoms through psycho-education and interventions, facilitate referrals to higher levels of care (e.g., ACT), assist clients with accessing benefits and resources, liaise between clients, families, and community resources, educate on available resources, deliver services in off-site/community settings, respond to crises, maintain compliant documentation (notes, treatment plans, progress reports), and collaborate with care teams. Compensation is salaried with a range contingent on licensure/experience. Qualifications include a master’s degree in Counseling or related field, NC licensure (LCSW/LCSWA/LCMHC/LCMHCA/LMFT or Licensed Psychologist), minimum two years’ relevant experience, knowledge of NC human services delivery, and valid driver’s license with auto insurance. Desired Qualifications
