Job Summary
Community Support Team Lead responsible for providing community-based therapy and case management to adults with serious mental health challenges. Conduct initial assessments and reassessments, provide individual/family/group therapy using evidence-based practices, develop person-centered service plans, monitor symptoms, facilitate referrals to higher levels of care (e.g., ACT), assist with benefits/resources coordination, liaison between clients/families and resources, deliver services in off-site/community settings, respond to crises, maintain compliant documentation, and collaborate with care teams. Role includes supervision (if licensure level allows) and opportunities for clinical focus development; compensation is salary-based with benefits and advancement opportunities.
Required Qualifications
- Master’s degree from an accredited university in Counseling or a related field
- Valid licensure to provide mental/behavioral healthcare in North Carolina (LCSW, LCSWA, LCMHC, LCMHCA, LMFT or Licensed Psychologist)
- Minimum of two years’ experience with individuals with mental and/or behavioral health issues (including writing person-centered plans, conducting group therapy, providing mental health diagnoses)
- General working knowledge of the human services delivery system in North Carolina
- Valid driver’s license, auto insurance, and reliable transportation
