Job Summary
Provide 1:1 in-home and in-community support to individuals with developmental disabilities to promote independence, assisting with daily living activities (personal care, meal prep, transportation, housekeeping, laundry) and delivering skill development in areas such as community participation, health and safety, and adaptive skills. Responsibilities include accompanying individuals to errands/appointments, documenting progress and writing progress reports, attending team meetings, coordinating with supervisors, and fostering independence and meaningful daily routines. Requires reliable transportation and use of own vehicle for travel between shifts, with mileage reimbursement; role emphasizes coaching, mentoring, and positive support to help person-served achieve their goals.
Required Qualifications
- High school diploma or GED
- Associates in relevant field OR two years equivalent experience in working with developmentally disabled and/or mental illness, as determined by the supervisor.
- Valid driver's license with acceptable driving record, auto liability insurance, and a reliable vehicle to transport person-served
- Must be at least 18 or 21 depending on person-served
- Willing to drive a 30-mile radius
- Adept in the use of technology and possess a smart device with adequate data
- Good communication and documentation skills
