Community Support Specialist

Community Support Specialist focused on coordinating social services, developing individualized care plans with nurses and case managers, conducting on-site client visits, maintaining organized client charts, and delivering culturally appropriate health education and counseling to participants and caregivers. The role involves two to three days at the Braintree HQ and two days in the field, with responsibilities including psychosocial evaluations, supervising AFC caregivers alongside the RN and case manager, discharge planning, ensuring documentation compliance, and facilitating access to healthcare and community services. Requires High School Diploma, a valid driver’s license with insurance, and at least one year of clinical experience with elderly or disabled populations.
