Job Summary
Provide patient-centered behavioral health support, care coordination, and life-skills development to help patients navigate resources, appointments, and treatment plans. Facilitate individual and group-based support services and skills training; coordinate care with providers, clinical teams, and community organizations; maintain ongoing communication and follow-up for continuity of care; complete timely documentation and reporting; participate in team trainings and supervision; build positive, professional relationships with patients, families, coworkers, and community partners; promote a respectful, inclusive, and culturally responsive care environment. Requires HS diploma or equivalent, Colorado certifications or ability to obtain within one year, valid Colorado driver’s license with willingness to travel, and ability to pass a Behavioral Health background check. Preferred qualifications include a Bachelor’s degree in related fields and strong communication, organization, and problem-solving skills.
Required Qualifications
- High school diploma or equivalent
- Colorado Peer and Family Specialist Certification (CPFS), National Certified Peer Recovery Support Specialist Certification (NCPRSS), or ability to obtain certification within one year of hire
- Valid Colorado driver’s license and willingness to travel
- Ability to pass a Behavioral Health background investigation
