Job Summary
Deliver individualized and group support focused on recovery, wellness, and daily living skills using a strengths-based, person-centered approach. Assist clients in accessing essential resources such as housing, medical care, and education while maintaining timely documentation and completing required reporting. Conduct outreach and re-engagement efforts as needed, collect and process urine drug screens, and provide client transportation when clinically indicated. This full-time Monday–Friday position requires a valid driver's license and agency-established minimum automobile coverage, with travel and driving being essential functions.
