Job Summary
Deliver individualized and group support focused on recovery, wellness, and daily living skills; work within a care team to connect clients with housing, medical care, education, and recovery supports; provide person-centered services; conduct outreach and re-engagement; collect and process urine drug screens; provide client transportation; travel is an essential function; documentation and reporting required; collaborate with clinicians, peer specialists, and healthcare providers; strong communication, problem-solving, and teamwork skills.
Required Qualifications
- High School/GED required
- Associate’s*/Bachelor’s/Master’s degree in a human service-related field preferred
- valid driver's license and automobile coverage required
- experience in delivering services to individuals with mental illness, substance use disorders, or developmental disabilities
