Job Summary
Develop and review client care plans by identifying goals, objectives, and necessary mental health services. Teach members independent living skills such as housekeeping, meal preparation, and coping strategies. Provide support to members with collateral social service agencies and the community while documenting all services in an Electronic Health Record. Drive clients to appointments, benefits offices, and grocery stores to remove barriers to care and strengthen daily living skills. This role operates Monday through Friday from 8:30am to 5:00pm, requiring a valid driver's license and willingness to transport clients. Required Qualifications
- To be at least 21 years old
- A valid driver's license
- Current car insurance
- Daily access to your own car
- Willingness to provide transportation to clients
- Must obtain and maintain liability car insurance of: $100,000 per person bodily injury, $300,000 per accident, $100,000 property damage
- High school diploma OR GED certificate AND 5 years of supervised clinical experience
- Bachelor's degree in Psychology, Social Work, or related field
- Bachelor's degree in an unrelated field AND 2 years of supervised clinical experience
- Master's degree in Psychology, Social Work, or related field
