Community Support Specialist

Job Summary
Provide compassionate, person-centered support to individuals experiencing behavioral health crises through in-person and telephone interactions in an outpatient urgent care setting. Engage clients during and following crisis visits to promote stabilization, encourage treatment participation, and support successful transition to ongoing behavioral health services. Build trusting relationships using trauma-informed and recovery-oriented approaches, assist clients in identifying barriers to care, and coordinate follow-up care including transportation to appointments using a personal vehicle. Document client interactions, referrals, and progress accurately within the Carelogic electronic health record while collaborating closely with therapists, nurses, and prescribers to ensure coordinated, high-quality crisis and follow-up care. Monitor client progress following intervention and communicate changes in needs to the treatment team to promote safety and continuity of care.
Required Qualifications
- Bachelor's degree in a related field, or four (4) years of equivalent experience in lieu of a degree
- Valid driver's license
- Current auto insurance
- A reliable, registered vehicle available for work use, including transporting clients
- Full-time position, consisting of three (3) 12-hour shifts each week, plus one (1) 8-hour shift every other week
- Urgent Care operates seven (7) days per week; candidates must be available to work rotating weekends and holidays as needed
Desired Qualifications
- Two (2)+ years of experience in a substance use, behavioral health, or related setting preferred
- Knowledge of community resources and health/social service systems in the Kansas City metro area preferred
