Job Summary
Conduct direct training and supports for individuals transitioning into housing, focusing on community integration, tenancy support planning, and rehabilitative independent living skills. Teach fire safety, emergency contact protocols, and daily living tasks including apartment maintenance, budgeting, and self-medication. Act as a liaison between residents, landlords, and treatment teams while developing Individualized Support Plans. Coordinate psychiatric and physical health services with Health Home and ensure compliance with mandated treatment orders. Complete all required documentation and maintain strict confidentiality.
Required Qualifications
- High School Diploma or equivalent
- At least 1 (one) full years of experience in Human Services or related field
- Proficient computer skills
- Proficient writing skills with grasp of basic grammar and spelling
- Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs
- Must be efficient, well organized, and have excellent communication skills
- Valid NYS driver's license and satisfy criteria to be a driver for FREE
- Must be able to lift 25 pounds at a time
