Job Summary
Assess patient needs and capabilities to link individuals to community agencies and resources as dictated by patient need. Conduct initial assessments, gather relevant data including proof of income and medication dosages, and document all information required for referrals. Coordinate services and resources to meet patients' ancillary needs based on specific referral requests from primary clinicians. Requires a valid MD Driver's License and clearance by the LBH Fleet Safety Department.
Required Qualifications

- Bachelor's Degree in Human Services or related field required
- Valid MD Driver's License required
