Community Support Specialist ITCD

Provide individualized, stage-matched recovery support to adults with co-occurring mental health and substance use disorders. Deliver services grounded in motivational and strength-based approaches, coordinate care across mental health, physical health, and substance use treatment domains, and support clients in achieving personalized recovery goals across all dimensions of wellness. Facilitate group services such as early recovery, WRAP, and family support, assist with crisis planning and discharge coordination, connect clients to housing, benefits, and employment resources, and provide transportation support as needed. Requires High School/GED; Associate’s/Bachelor’s degree in a human service-related field preferred. Valid driver’s license and agency-required automobile coverage. Strong collaboration with ITCD team and community resources to deliver holistic, integrated care.
