Job Summary
Community-based ACTT role providing recovery-focused supports: Partner with individuals to achieve recovery goals; conduct co-occurring screens (C-SSRS) and functional assessments; develop individualized, person-centered treatment plans; utilize motivational interventions; deliver psycho-educational rehabilitation via group and individual modalities; assist with housing/utilities, transportation planning, and family engagement; develop crisis and safety plans; and collaborate with staff to respond to crises while providing skills teaching and community integration.
Required Qualifications
- Bachelor’s degree or equivalent education and/or work experience required
- 1+ years working with identified population preferred
- Valid driver’s license required
