Job Summary
Know the service plans and safeguards for each individual served, and assist in developing and evaluating service plans based on individual preferences. Provide safe transportation using a reliable vehicle and driver's license, and back up the cook as needed, including driving a food truck when possible. Learn the eVero database system for recordkeeping, write daily and monthly notes, and teach techniques to help individuals achieve their goals. Support activity participation in the community, assist in selecting appropriate community activities, and help individuals develop positive interpersonal relationships. Attend required OPWDD and agency trainings, staff meetings, and complete other duties as assigned.
Required Qualifications
- Possession of a high school or a high school equivalency
- Successful completion of the Direct Support Assistant Traineeship
- Valid NY State Driver License
- Access to a reliable vehicle
- Ability to sit

- Ability to walk
- Ability to use hands to type
- Ability to perform light lifting
- Close vision

- Ability to adjust focus
- Adherence to all applicable federal and state laws and regulations including confidentiality, privacy, program, billing, and documentation standards
