Job Summary
Provides community-based/in-home behavioral health services for individuals experiencing serious mental illness that interferes with their ability to manage mental health symptoms and live independently. Includes transportation to client, assessment and treatment planning, crisis intervention, and coordination with community agencies; supports clients with daily living activities, benefits access, employment/education, advocacy, and resource navigation, while maintaining documentation and adhering to regulatory standards. Works as part of an interdisciplinary team to meet clinical productivity and compliance expectations, with after-hours on-call coverage as assigned.
Required Qualifications
- Bachelor's degree in counseling, Social Work, LPN, related field OR H.S. Diploma/GED with a minimum of five years documented clinical experience in mental health or human services.
- Registered Professional Nurse (LPN) License Illinois - If degree is in Nursing Upon Hire
- Valid Driver‘s License - Illinois Secretary of State.
