Community Support Specialist (ACT-TAY)

Job Summary
Provide community-based mental health services to children, adolescents, and adults, including assessment, diagnosis, treatment planning, and intervention implementation. Collaborate with clients, families, and stakeholders to develop individualized treatment plans using evidence-based approaches like Motivational Interviewing, CBT, and DBT. Coordinate care with physicians, schools, and other providers while maintaining accurate client records and documentation. Prepare for and facilitate group therapy sessions as part of your assigned caseload in clients' homes, clinics, and community locations. Participate in staff training, quality improvement initiatives, and team collaboration to support client recovery and resiliency.
Required Qualifications
- Bachelor's degree in Psychology, Social Work, human services field or equivalent experience
- Background check
- Training
- Valid driver's license
- Reliable transportation
- Specific skills/knowledge in case management
- Specific skills/knowledge in mental health
- Specific skills/knowledge in substance use
- Specific skills/knowledge in documentation
- Bachelor's degree in related field
- Associate's in Behavioral Health
- 4 years qualifying experience in mental health/substance use services delivery
- Knowledge of ACT model for some roles
