Job Summary
Conduct crisis intervention and case management for youth with behavioral health disabilities across Houston, Peach, and Crawford Counties. Serve as a positive role model while identifying community resources for social, economic, and educational needs. Manage medication compliance, address aggressive behavior to ensure safety, and provide behavior management training. Act as a liaison with agencies, professionals, and government officials; monitor client progress and treatment plan effectiveness. Assist clients with appointments and provide transportation, adhering to strict vehicle safety policies. Maintain accurate documentation, participate in interdisciplinary team meetings, and contribute to program evaluations. Manage a caseload of up to 50 clients while meeting minimum billing requirements.
Required Qualifications
- High School Diploma or GED
- Valid, Unrestricted Georgia Driver's License
- Must be computer literate
- Transporting individuals served is a requirement
Desired Qualifications
- Bachelor's Degree in related field
- two (2) years of experience working with youth and/or individuals with Behavioral Health disabilities
