Job Summary
Conduct crisis intervention and case management for youth with behavioral health disabilities across Webster, Schley, Macon, Taylor, and Sumter Counties. Provide individualized support, including behavior management training, skill building, and transportation to appointments, while ensuring medication compliance and safety. Act as a liaison with schools, agencies, and community professionals to identify services and monitor treatment plan effectiveness. Maintain accurate documentation, participate in interdisciplinary team meetings and supervision, and contribute to program evaluations. Manage a caseload of up to 50 clients while meeting minimum billing requirements and operating office equipment.
Required Qualifications
- High School Diploma or GED
- Valid, Unrestricted Georgia Driver's License
- Must be computer literate
- Transporting individuals served is a requirement
Desired Qualifications
- Bachelor's Degree in related field
- two (2) years of experience working with youth and/or individuals with Behavioral Health disabilities
