Job Summary
Community Support Facilitator responsibilities include providing group and/or individual support in the community as identified in the consumer's Individualized Personal Plan, developing opportunities for participation, facilitating and modeling interactive relationships, traveling with and/or transporting clients, advocating on behalf of consumers, completing required documentation (forms, incident reports, IPPs), maintaining professional community representation, implementing behavioral intervention plans when needed to ensure safety and learning, coordinating consumer programming with agency activities and staff, and acting as a mandated abuse reporter. The role operates in the Gilroy/Hollister area with extensive community travel guidance, and requires the ability to work independently and cooperatively with a diverse population. Knowledge of normalization principles, assistive technology, strong communication skills, and CPR/First Aid certifications are valued; bilingual or alternative communication support skills are a plus. Required Qualifications
- High school diploma, GED, or equivalent, plus two years of related experience or an approved combination of experience and education.
- Possession of a valid California drivers license and clear driving record.
