Job Summary
Deliver community support services to children and adults enrolled in the CPRC program, managing a caseload of 20–25 clients through weekly meetings in homes or residential centers. Provide training and education on daily living skills, monitor treatment plan goals, and refer clients to community resources to improve functionality. Maintain accurate documentation of all client interactions per agency guidelines and clinical policies. This floater position requires travel to any of five Missouri offices based on staffing needs, with a Monday–Friday schedule from 8:00 am to 4:30 pm.
Required Qualifications
- Bachelor's Degree
- Any four-year combination of higher education and qualifying experience
- Four years of qualifying experience
- Qualifying experience includes service delivery to individuals with mental illness, substance abuse disorders, or developmental disabilities
- MO Class E Driver's License (Class D for IL Residents)
- Proof of continuous auto insurance
- Must meet CCC's Motor Vehicle Record standards
- Reliable transportation
- Travel in own vehicle
