Job Summary
Deliver community support services for assigned clients within the CPRC, managing a caseload of 20-25 clients and meeting with them weekly in community settings such as client homes or residential centers. Provide training/education to clients in daily living skills, document interactions per agency guidelines, and coordinate with the treatment team to monitor and support treatment plan goals. Refer clients to appropriate community resources to improve functionality and develop a support system, and ensure documentation adheres to clinical policies and standards (DMH, CARF, and federal/state). This role supports adults and children with disabilities in social role functioning and daily living skills within a Behavioral Health network, with benefits including PTO, holidays, health/dental/vision insurance, life and disability coverage, retirement auto-enrollment, PSLF eligibility, and referral bonuses.
Required Qualifications
- Bachelor's Degree or higher
- Four years of qualifying experience, or any four-year combination of higher education and qualifying experience
- Qualifying experience includes service delivery to individuals with mental illness, substance abuse disorders, or developmental disabilities
- Must hold or obtain by start date MO Class E Driver's License (Class D for IL Residents)
- Proof of continuous auto insurance
- Travel in own vehicle is required
