Job Summary
Community Supports Case Manager I-III oversees all case management activities for assigned programs including Victim Intervention and housing resources. Provides client-centered support to help clients access victim services, housing navigation, and related social services. Responsibilities include intake, assessment, care planning, linkage and coordination with community resources; monitoring charts monthly for compliance; ensuring timely, accurate documentation; maintaining confidentiality and HIPAA compliance; and contributing to trauma-informed program development to meet client needs.
Required Qualifications
- Education and Experience: High School Diploma with 2 years of experience or a bachelor’s degree with 1 year of experience or a master’s degree in social work or related field with internship plus six or more months experience in Case management.
