Job Summary
The Community Specialist provides support across work, home, school, or community settings to individuals receiving services and their families, helping them achieve personal goals outlined in individual goal plans and offering periodic relief to caregivers. Responsibilities include assessing hopes and desires, promoting rights and advocacy, connecting individuals to community resources, supervising and mentoring staff, maintaining required documentation and safety, and participating in meetings and trainings. The role may involve driving and travel, requires at least a high school diploma and a valid US driver’s license, and is based in North Carolina with the employer Monarch. The position emphasizes person-centered, positive relationships, and documentation of progress, actions toward outcomes, and incidents, per agency policies and state and federal regulations.
