Job Summary
Community Services Case Manager position focused on delivering pregnancy support, rent/utility assistance, Thrive Life Skills, human trafficking victim assistance, and homelessness services. Responsibilities include direct case management across multiple programs, home/office/community visits, creating case plans, crisis management, referrals, client education (budgeting, FAFSA, post-secondary enrollment), service coordination with providers, grant data collection, and community representation. Requires Bachelor’s degree in social services or related field, 1+ year social service experience, knowledge of CORE 40/HSE, FAFSA processes, financial literacy, ability to travel within the West Region, and bilingual Spanish preferred. Must maintain ethics and confidentiality, with valid driver’s license and reliable vehicle. Occasional travel and documentation duties, plus compliance with agency policies.
Required Qualifications
- Bachelor’s degree in social service or related field from an accredited college/university
- minimum of one year work experience in social service setting
- Working knowledge of community service and public program providers
