Job Summary
Community Service Provider role delivering home- and community-based services to children with emotional and mental health challenges. Travel locally to work with children and families, providing rehabilitative services including non-licensed counseling, youth and family support, caregiver support, advocacy, pre-vocational services, and respite within a Family-driven Care framework. Coordinate with families on treatment goals, implement services per the Treatment Plan, assist with scheduling and service delivery, attend required trainings, and complete all documentation. Requires a Bachelor's degree (with 2+ years of related experience) or a Master's degree (with 1+ year of related experience) and a valid NYS Driver's License. Required Qualifications
- Bachelor's degree in a related field and at least two years of related experience OR Master's degree in Social Work, counseling, psych, or related human service with at least one year of related experience
